Welcome to Somis School!
All new students to our school MUST complete this online pre-enrollment.
Registrations for Transitioal Kindergarten through 8th grade for school year 2022-2023 is open. Students enrolling in Kindergarten must be five years of age by September 1, 2022 (EC 4800a); students with birthdates between September 2, 2017 and December 2, 2017 are eligible to enroll for Transitional Kindergarten (TK).
Please bring or upload the following documentation to register:
1. Verification of Student Date of Birth:
✓ Original Birth Certificate, Passport, Certified Hospital Birth Record, or duly attested Baptismal Record.
2. Verification of Immunizations - Bring the immunization record at the time of enrollment. Your child must have all vaccinations needed. The following immunizations are required for school entry:
✓ 5- DTap, 4- Polio, 3- Hepatitis B, 2- MMR, 2- Varicella.
✓ Tdap is required for students enrolling in the 7th or 8th grade.
✓ Health Examination For School Entry Required.
3. Proof of Residency (TWO of the following is required) - Document must be dated within 30 days :
✓ Gas, Water, Electric, Telephone, rent receipt or a lease agreement. Bill must have parent’s name and date on the document. If unable to provide proof of residence, ask for an Affidavit of residence and return with a proof of address of person who signed the affidavit.
4. Legal Custody papers – if there are any legal court orders or foster placement papers.
5. Special Education:
✓ If your child has a 504 Plan or an IEP Individualized Education Program or any other specialized plan
6. Physical and Oral Health Examination:
✓ Is required for students enrolling only in TK and Kindergarten
